This website is not for medical information

The scheduling form on this site intentionally collects contact and scheduling details only: name, phone, email, general procedure interest, and preferred time. Please do not submit health details, medical history, or photographs through this website or by ordinary email. Health information belongs in the consultation room, where it becomes part of your medical record and is protected accordingly. Submitting the form does not create a doctor–patient relationship.
