Who this operation actually helps.

Rhinoplasty is one operation with two jobs. Some patients come for shape: a dorsal hump, a tip that droops in photographs, a bridge that reads wider than the rest of the face. Some come because they cannot breathe through one side, especially at night. Many come for both, and the two are planned together, because changing shape without respecting the airway trades one problem for another.

Good candidates are done growing (typically 16 and up), are generally healthy, are not smoking around surgery, and want a specific, describable change. The strongest predictor of satisfaction we see is a concrete goal: a photograph angle you avoid, a breathing test you fail. If your goal is a different face, rhinoplasty is not that operation, and Dr. Marlowe will say so in consultation.

Realistic expectations are part of the plan itself: your skin thickness, cartilage strength, and healing pattern set the range of what surgery can hold. That range is discussed with you before anything is scheduled, with your own photographs on the screen.

Two ways into the same operation. The choice follows your anatomy and your plan; you will know which is recommended for you, and why, before you decide anything.

T-01

OPEN APPROACH

A small incision across the columella, the strip of skin between the nostrils, lets the skin lift so the framework is fully visible. Chosen when tip work is significant or grafts need precise placement. The trade: a fine external scar, typically faint by a few months, and somewhat longer tip swelling.

T-02

CLOSED APPROACH

All incisions stay inside the nostrils. Chosen for hump reduction and conservative reshaping where full exposure is not needed. The trade: no external scar and often a quicker-settling tip, but less direct visibility, so it suits some plans and not others.

FUNCTIONAL PAIRING · SEPTOPLASTY

When a deviated septum restricts breathing, septoplasty is performed in the same operation: one anesthesia, one recovery. When the functional portion is medically indicated, it is documented separately, and it may be billable to your health insurance; Dana reviews that with you before surgery, in writing like everything else.

SPLINT OFF

The splint comes off

At the one-week visit the splint and any external sutures are removed. Expect swelling and bruising around the eyes in week one; congestion without packing, since we use soft internal splints instead.

Email and calls from home are typical within a few days.

WK 02

PRESENTABLE

Back in the room

Most visible bruising resolves around the two-week mark; what remains usually covers with concealer. Most patients are back at work and social plans, with the swelling that remains reading as fullness, not surgery.

EXERCISE CLEARED

Training resumes

After the six-week check, cardio and lifting typically resume. Until then, keep glasses from resting on the bridge; we show you taping and cheek-rest workarounds at the day-7 visit. Contact sports wait longer, cleared case by case.

FINAL REFINEMENT

The result settles

Residual swelling leaves the tip last, and slowly. Subtle refinement continues up to twelve months, which is why revision decisions are never made early. The photograph at one year is the honest one.

Does it hurt?

Less than most patients expect. The first days feel like a heavy head cold: pressure and congestion more than pain. Most patients use prescription medication for a day or two, then plain acetaminophen. The congestion improves noticeably once the internal splints are out at the one-week visit.

Will my nose be packed?

No gauze packing in the traditional sense. We use soft internal splints that hold the septum quietly and come out at the day-7 visit, together with the external splint. Removal takes seconds and is more strange than painful, which is exactly how patients describe it afterward.

When will I see the result?

In stages. At two weeks you look presentable to other people; you will still see swelling yourself. Much of it resolves over the first six weeks, and the tip refines slowly for up to twelve months. That is why your one-year photographs, not your one-month mirror, are the fair comparison; individual healing varies.

What if I need a revision?

Rhinoplasty carries a real revision rate; nationally, figures between 5 and 10 percent are commonly cited. Two protections are written into your quote: no revision decisions before the twelve-month mark while swelling still moves, and if Dr. Marlowe recommends a revision within the first year, no surgeon’s fee, with facility and anesthesia at cost.
